The Central Records Office is proud to serve the residents and visitors of the Town of Falmouth.

Alarm System Registration Form
Town of Falmouth by-laws require that all residential and commercial burglar alarm systems be registered with the police department (Town Code Chapter 75 Alarm Systems). First time registration is $25 and must be updated annually. The Alarm System Registration form may be picked up at the police station or downloaded. Please mail completed forms to the Central Records Office, 750 Main St., Falmouth, MA 02540 or fax completed forms to 508-457-2566.
Business Information Form
Complete this form if you have a business in Falmouth, then submit it to the Falmouth Police Department in case there is an emergency.
Disabled Placard/License Plate
Disabled Placards or License Plates are only available from the Massachusetts Registry of Motor Vehicles. The Disabled Placard/License Plate application can be picked up in person in the Central Records Department, or can be downloaded here in Adobe Acrobat format. You will need Adobe Acrobat Reader to view/print this form. If you do not have Acrobat on your computer, download it free by following this link. Individuals requesting a Town of Falmouth Hawker/Peddler permit can pick up an application for the Commonwealth of Massachusetts Hawker/Peddler permit at the Central Records office. Once the state permit is issued, the applicant will need to return to the Falmouth Police Department and register to conduct business in Falmouth. Local registration requires that the applicant have two (2) 1″x 1″ color photos and a check or money order made out to the Town of Falmouth in the amount of $10.00 (check or money order only).

Taxi/Livery/Limousine Operator Licensing (Vehicle for Hire)
Individuals requesting a Town of Falmouth Vehicle for Hire Operators permit must apply at the Central Records office. Applicants will need to have a letter of employment, two (2) 1″x 1″ color photos and a check or money order made out to the Town of Falmouth in the amount of $25.00.

[Woods Hole] Falmouth Resident Parking Permits
Falmouth Resident Parking permits are available for Falmouth residents and property owners from the Central Records office. A current vehicle registration (with a Falmouth address), property tax bill, or other suitable identification will be required in order to obtain a permit. Cost for the parking permits is $15 per vehicle (check or money order only). The permits are issued annually, expiring December 31 of each year.
